South West Individual Support provides face to face, professional support for NDIS participants in Support Co Ordination, Support Work and Community Activities.
We support you from Geelong to Warrnambool areas throughout Victoria.
We have a very friendly, professional and experienced team with extensive knowledge of all aspects of NDIS including gaining access to the scheme and planning processes, Child Protection, Mental Health, Nursing, Adult and Child disabilities.
South West Individual Support are inclusive of all ages, backgrounds and disabilities and can work with plan managed or self-managed NDIS participants.

Support Coordination
We use a “best fit” approach when connecting our participants with providers. We offer choice and control every step of the way. We monitor the participants funding allowing them the best possible outcomes for their NDIS plans. We are a reliable, innovative and flexible organisation always putting our participants first.
Support Work
Our participants are paired with their Support Worker based on their personalities and NDIS goals. They build a relationship with both the participants and their families. They perform all duties in a professional and caring manner, assisting the participant in their day to day lives
Community Activities
We not only assist our participants to access the community individually but also in a group setting. We keep up to date with the latest events happening around the community and encourage our participants to attend our group programs to build on their social skills.
