Support Coordinator

Leonie has been working with people with disabilities from 1980. Through her time, she has worked in residential facilities, completed her training as a specialised disability nurse, has attained a graduate certificate in Mental Health, and completed management training. She has managed Supported Independent Living homes at DFFH and Home@Scope and worked as an operations manager in this field. 

 During her years she has worked extensively with people with intellectual disabilities who have the additional challenge of Mental Illness. She has worked with people who have behaviours of concern. 

 Leonie is very committed to having people actively engaged in living, making decisions about their own lives as much as possible, and teaching and supporting staff to work with people respectfully and with dignity.

Support Coordinator

Christine has worked for 7 years in early years programs through Barwon Child Youth and Family, continuing onto providing administration and therapy assistant support at Talk to Me Speech Pathology. She then transitioned to Local Area Coordinator for the NDIS in 2020 before becoming a Support Coordinator in 2022.

Her lived and work experience has involved family support, early intervention, group programs, mental health support, local knowledge and the NDIS processes.

Christine enjoys being able to see the impact supports can have in people’s lives, navigating any concerns and working out different ways to provide essential support to my participants to ensure they can enjoy their daily life.

Support Coordinator

Josh has extensive NDIS & disability sector knowledge and previously worked as a Local Area Coordinator in the NDIA. His previous roles have seen him working in the defence force for 6 years. He has been able to use skills learned there to support his participants as a Support Coordinator.

Josh is passionate about working with participants to achieve their goals and to be able to live their best life.

Support Coordinator

Darci has spent 20 years working in the community services field, in youth work, housing, community development and the disability sector. She has a degree in Psychology and have experience dealing with people with mental health issues. In 2020 Darci started working for the NDIS as a Local Area Coordinator and in 2022 moved into the support coordination field.

Darci enjoys being a Support Coordinator as it gives her the opportunity to work with people to help them achieve their goals.

Darci likes meeting people with varied experiences and life stories and value the opportunities she is given to walk beside someone and see them grow

Support Coordinator

Kris has an extensive background working with people with disabilities and complex needs.

Kris worked in disability nursing for over 26 years before transitioning to a coordinator role. After which Kris transitioned into a family support role and working with individuals with homelessness, for 16 years.

Kris joined the SWIS team in 2023 as a support coordinator.

Kris has worked with a very diverse and extensive range of support needs and situations. Kris is passionate about advocacy and providing quality care and support, ensuring everyone has a voice about their own unique situation.

Support Coordinator/Admin

Shanae has been working is the disability field for over 6 years. Shanae commenced work as a support worker within a residential facility, before branching out into community engagement.

Shanae is a dually qualified registered Nurse and Registered Midwife, with additional certificates in neonatal care. Shanae has a special interest in working with women and children and is passionate about advocating for the rights of all people.

Shanae has stepped into the role of support coordinator December 2023, and is committed to helping participants achieve their goals.

Shanae also assists with company administration needs, enquires and organising community and social programs. She will be on extended leave from March 2025.

Support Coordinator

Bernadette has been working in the health field for 20 years. As an RN she has several years’ experience in aged care, community and primary health care settings. She enjoys meeting people in the local community and helping participants work towards their goals.

Bernadette works with a number of participants who have complex medical needs.

Support Worker

Holly grew up in the local area and has worked as an enrolled nurse for the past 16 years. She joined South West Individual earlier this year as a support worker.

Holly thoroughly enjoys her role, she enjoys meeting new people and getting to know and support her participants and their individual needs.

Support Worker

Fraser is an experienced Disability Support Worker who has worked with people in their homes for the last five years. He is very passionate about enabling people to exercise their choice and control.

When not working, Fraser is interested in gym work, jet skiing, camping with mates, playing pool, and experimenting in the garden.

Fraser has a friendly, happy, relaxed personality whose talent lies in making people feel calm.

Support Worker

Raz grew up in the local area. Raz is a qualified pastry chef and has her Certificate 4 in childcare. Raz is also well experienced with caring for foster children.

Raz had spent eight years at Colac Area health in a supervisor role before joining SWIS in Feb 2023.

Raz absolutely loves her support work role and meeting new people from within the community. You’ll find Raz in the SWIS kitchen, cooking up a storm with participants during the SWIS cooking programs.

In her spare time, Raz keeps busy spending time with her partner and children.

Support Worker

Lucinda has worked with people with disabilities for more than 35 years, specialising in supports for people with complex communication needs and behaviour supports.

Lucinda has worked across age groups from 6 months old to 60+ years.

Lucinda has training and support practice experiences across Autism spectrum, Trauma informed practice and dual disability.

Lucinda engages with individuals to build and enhances relationships with people to maximise their opportunities within their community to achieve their goals.

Lucinda holds expertise with skill and capacity building and specialises in person centred practice.

Support Worker

Bronte grew up in the local district where she completed year 12 in 2023. Bronte is currently continuing her studies at The Gordon, completing a Certificate 4 in Beauty Therapy.

Bronte works part time as a support worker and thoroughly enjoys meeting new people and working within the community.

In her spare time, you’ll find Bronte on the netball court, at the speedway or spending time with her family.

Support Worker

Alan is a very experienced Disability Support Worker who has worked with people in the community and their homes for the last four years. He is very passionate about supporting people to be fully engaged in their own lives. He likes to support people to achieve their goals.

When not working, Alan is interested in fishing, camping, four-wheel driving, footy and sports.

Alan has a friendly, fun, relaxed personality. He is great at relating to people in a friendly and fun way. He can relate well to people from all walks of life.
